If you would like a COVID-19 Vaccine Certificate (Vaccine Passport), please apply through the Nagano City Health Center’s Health Care Section.

Individuals who were vaccinated in Nagano City can receive a Vaccine Certificate at the Nagano City Health Center.

Please send in the necessary documents, or bring them in directly to the Health Center to apply.

【Necessary Documents】

◎For use in Japan only 『Vaccine Certificate for Domestic use in Japan』

１. Application Form 373997_1.pdf (city.nagano.nagano.jp)

２. Copy of your 『COVID-19 Vaccine Completion Certificate』or 『COVID-19 Vaccine Record』

　　Please copy the certificate or record (with date & information stickers pasted on them) that was given to you when you received the vaccine.

３. Copy of document to verify your identity

　　Please make a copy of one of the following forms of ID:
　　　Foreign Residence Card, Driver’s License, Health Insurance Card, etc.

４. Envelope ()

　　Paste an ￥84 postage stamp and write your name and address on the envelope.

◎For international travel as well as domestic use 『Vaccine Certificate for International Travel and Domestic use in Japan』()

２. Copy of your 『COVID-19 Vaccine Completion Certificate』or『COVID-19 Vaccine Record』

　　Please make a copy of the certificate or record (with date & information stickers pasted on them) that was given to you when you received the vaccine.

３. Copy of a currently valid passport

　　Please make a copy of the page that includes your name and passport number.

４. Copy of document to verify your identity

　　Please make a copy of one of the following forms of ID:
　　　Foreign Residence Card, Driver’s License, Health Insurance Card, etc.

５. Envelope()

　　Paste an ￥84 postage stamp and write your name and address on the envelope.
