For those who have received a COVID-19 Vaccine Voucher in the mail
For those who have received a notice in the mail about the COVID-19 vaccine, make sure you have the vaccine voucher and the Medical Examination Form. Please make an appointment at a hospital and receive the vaccine.
Refer to one of the foreign language versions of the Medical Examination Form to fill out the Japanese version.
